During the evaluation phase, nurses should ask several key questions:
Has the patient's condition improved, deteriorated, or remained the same? Have the set goals and outcomes been met? Were the nursing interventions effective? If not, why? Are there any new issues that need to be addressed? Is there a need for a change in the care plan or additional interventions?
