- Medication errors: Wrong dosage, incorrect medication, or missed doses. - Falls: Any instance where a patient falls, regardless of injury. - Infections: Hospital-acquired infections such as MRSA or C. difficile. - Pressure ulcers: Development of bedsores or pressure injuries. - Equipment failures: Malfunctioning medical devices. - Patient complaints: Any patient-reported issues that indicate potential safety concerns.
