What Constitutes Protected Health Information (PHI)?
Protected Health Information (PHI) includes any information that can be used to identify a patient and relates to their health status, treatment, or payment. Examples of PHI include names, addresses, birth dates, Social Security numbers, medical records, and insurance information. Nurses must be vigilant in protecting all forms of PHI, whether spoken, written, or electronic.
