Falsifying Patient Records: This involves adding or altering information in patient records to misrepresent the care provided. Upcoding: Billing for a higher level of care than was actually provided. Ghost Patients: Creating records for patients who do not exist to bill for services that were never rendered. Duplicate Billing: Billing multiple times for the same service. Unnecessary Services: Documenting and billing for services that are not medically necessary.
