The efficacy of a drug is determined by the concentration of its free (unbound) form, which is able to interact with target tissues or receptors. For example, if a drug is 95% bound to proteins, only 5% is available to exert a therapeutic effect. Changes in protein levels (e.g., due to liver disease or malnutrition) can therefore affect drug efficacy.
