The SF-36 is a widely used health survey questionnaire designed to assess overall health status and quality of life. It consists of 36 questions, which are grouped into eight scales: Physical Functioning, Role Limitations due to Physical Health, Role Limitations due to Emotional Problems, Energy/Fatigue, Emotional Well-being, Social Functioning, Pain, and General Health Perceptions.
