Documentation of pupil reactivity should be precise and include details such as the size of the pupils before and after light exposure, the speed of the reaction, and any asymmetry between the pupils. This data should be documented in the patient’s medical record and communicated to the healthcare team. Interpreting the score involves understanding that a score of 2 (normal reactivity) is generally a positive sign, indicating intact neurological function. A score of 0 (no reactivity), however, may indicate severe brain injury or other critical conditions requiring urgent medical attention.
