Each item on the HDRS is scored based on the severity of the symptom: - 0: Absent - 1: Mild - 2: Moderate - 3: Severe - 4: Very Severe
The total score is then calculated to determine the overall severity of depression: - 0-7: Normal - 8-13: Mild Depression - 14-18: Moderate Depression - 19-22: Severe Depression - 23+: Very Severe Depression
