Problem: This is the diagnostic label that describes the essence of the patient's response to health conditions. Etiology: This part identifies the cause or contributing factors, often stated as "related to" (R/T). Symptoms: These are the defining characteristics or signs and symptoms, often stated as "as evidenced by" (AEB).
